Effective August 9, 2004, Dr. Cornelison's license was revoked; his revocation was stayed and he was placed on 4 years of probation for practicing Optometry without a valid license and for fraudulently altering his license to appear unexpired. Subsequently, Dr. Cornelison failed to comply with the terms and conditions of his probation and a petition to revoke his probation was issued on August 8, 2008. Dr. Cornelison's license is revoked; revocation is stayed and his probation has been extended for three years with various terms and conditions.

Tom, Gregory Lawrence (OPT 10427)

On April 3, 2008, Dr. Tom's license was surrendered to the Board of Optometry on the grounds of insurance fraud. Dr. Tom petitioned for reinstatement of his license and his petition was granted. PETITIONS FOR REINSTATEMENT

Chan, Wyman (OPT 5017)

Effective October 27, 2010, the Petition for Reinstatement is denied. However, the denial shall be stayed upon successful completion of a Conditions Precedent as specified in the order. Once the Conditions Precedent has been successfully completed, the stay of the denial shall take effect, and Respondent's license shall be placed on probation for a period of 3 years, subject to certain terms and conditions. The stay of the denial took effect on September 12, 2012, and Respondent's license was placed on probation.
